Requirements and Guidelines from St. John's

Getting COVID-19 Vaccine Shots for Yourself and Your Family?

-Only Pfizer vaccines are available for children ages 5-17.
-Children 5-15 must be accompanied by a parent or the parent’s designee who must be 18 years or older.
-Children ages 16 and 17 need a signed waiver from their parent; however the parents or guardians do not need to be present and the children do not have to be accompanied by any other adult. Waiver is available online.
-Boosters are currently only approved for ages 18+.
-Moderna and Pfizer booster will be available.
-Persons without auto-immune issue may mix and match booster types. 
-Persons with auto-immune issues may not mix and match booster types.
-Moderna and Pfizer second doses will be available.
-No J&J vaccine available.
-No J&J booster available. 
-For individuals who received the J&J vaccine, mix and match is available for a booster after 2 months of initial dose.

Verification of COVID-19 Vaccination

The City of Los Angeles (City) Ordinance No. 187134 requires all City employees to be fully vaccinated and to report vaccination status.

Employees fully vaccinated through an LADWP sponsored LAFD or St. John’s Well Child and Family Center events held at JFB do not need to report any information at this time. LADWP has already reported this information as required, while maintaining employee confidentiality.

Requesting for Exemption from the COVID Vaccine

Ordinance No. 187134 also provides City employees with the ability to request a medical or religious exemption to be evaluated on a case-by-case basis. The impact of the Ordinance, including the exemptions process and non-compliance, is currently being discussed with our labor and management partners.
