Vaccines and Boosters for ALL LADWP employees and family ages 5+ at JFB
Please use the following links (Moderna or Pfizer) to pre-register for the event. Pre-registration strongly encouraged. No J&J. Walk-ins welcome. Bring your COVID-19 Vaccination Record with you to your appointment.
Second boosters offered for patients who are immunocompromised or 50 years and older. Information from St. Johns:
-Patients ages 50 years and older are now eligible for a 2nd booster, 4 months after their 1st booster.
-Immunocompromised patients are able to get a 2nd booster as well after 4 months.
-Immunocompromised patients should have 5 vaccines in total if they are up to date (3 primary doses and 2 boosters).
-However, for the immunocompromised patients, Pfizer remains to be for 12 years and older, and Moderna strictly for 18 years and older only.
-Boosters are 0.3ml for Pfizer and 0.25ml for Moderna.

Verification of COVID-19 Vaccination
The City of Los Angeles (City) Ordinance No. 187134 requires all City employees to be fully vaccinated and to report vaccination status.
To provide vaccination status to LADWP:
-Go to https://chsapp/Vaccination/, accessible on any
department computer or while connected to the LADWP
network.
-Schedule an appointment with the Resource Office. Email
[email protected] to schedule.
Employees fully vaccinated through an LADWP sponsored LAFD or St. John’s Well Child and
Family Center events held at JFB do not need to report any information at this time. LADWP
has already reported this information as required, while maintaining employee confidentiality.

Requesting for Exemption from the COVID Vaccine
Ordinance No. 187134 also provides City employees with the ability to request a medical or religious exemption to be evaluated on a case-by-case basis. The impact of the Ordinance, including the exemptions process and non-compliance, is currently being discussed with our labor and management partners.
